Summary:

The Creating Connections programme started in April 2013 as a two year Sport England Inclusive Sport funded project to design and develop a sport and physical activity pathway for disabled people across West Yorkshire.

Creating Connections uses a behavior change intervention that has been  designed to provide a structured approach to identifying and encouraging disabled people to become more active.

The primary outcome was to encourage disabled people (aged 14+) to increase their levels of physical activity. The secondary outcomes included encouraging disabled peopleto get involved in community sport and physical activities while also supporting community sports clubs and providers to welcome more disabled people to their sessions.
